Are your community rescue services prepared and properly trained in the event
of a catastrophic bus crash?
Motor vehicle crashes are the leading cause of death and disability.
Surviving a motor vehicle crash is dependent on the severity of the crash and
the injuries incurred, but it is also linked to the response time by skilled
rescue teams and a well-coordinated trauma care system.
The recent Huntsville, Alabama school bus crash that nose dived 30 feet below
an overpass made me stop and reflect on bus crashes in our own state. For
example: A tractor trailer in Rosendale came out of the fog smashing into
the car, shoving it under the school bus and plowing into the back of the bus
killing 5 and injuring 11 children…A driver in Green Bay carrying 39
students drives into the path of a semi truck and all the children were rushed
to three hospitals…A van crossing the center line in the town of Ixonia
crashed into a school bus carrying an elderly group, killing 4 passengers and
injuring 19…A motorcoach carrying Chippewa Falls School students and
their chaperones home from a band competition collided with a semi truck killing
5 and injuring 29…teen dies after crash with school bus in New Berlin
…tour bus overturned in Oak Creek, 15 people were injured and two were
seriously injured…bus versus truck crash in Waupun kills 1 and injures
8…A deadly bus crash in Waupaca County killed two people and injured more
than 30 when a SUV collided with a tour bus which rolled on its side into a
ditch…A tractor trailer in Plainfield smashed into the rear of a school
bus and tore open the back of the bus, injuring 6 children and both drivers who
were taken to the hospital…the list of tragic bus crashes goes on and on.
You would expect the rescuers who arrive on the scene to be trained and educated
on vehicle extrication. Sadly, this is not the case. There is NO certification
or standard extrication training for rescuers in the event of a school bus or
motorcoach vehicle crash. The current “get them out any way you can” is
unacceptable. This is not what I want for you, my family, friends or myself.
Removal of entrapped victims by inexperienced or undertrained rescue personnel
can result in delays and quick access to treatment; thereby reducing
survivability. In 2001, I sent a letter to the former state representative, Marc
Duff (http://www.besafeinc.org/motorcoach.htm),
addressing the need for rescuers to be properly trained to rescue victims from
bus crashes. To date, nothing has been done to improve the situation and
Wisconsin citizens, traveling by mass transit vehicles, continue to be at risk
on our roadways.
According to the U. S. Department of Transportation, the current National
Standard Curriculum for EMT’s does not include mass transit vehicle extrication
training. On the local level, the Wisconsin Administrative Code requires that
all firefighters be trained to meet the minimum standard which is called
“Firefighter 1”. At this level, the subject of extrication is never addressed.
“Firefighter 2” training consists of three hours devoted to hands-on automobile
extrication training with NO training for school bus or motorcoach crashes.
Thus, firefighters in Wisconsin are allowed by law to perform all duties
including motorcoach or school bus rescue without being formally trained or
certified. It is inconceivable to think that in today’s society, rescuers are
performing life saving vehicle extrications without a formal certification
program in place. Emergency personnel must be properly trained to save those
precious seconds that can mean the difference between life and death.
SAFE
provides rescuers with knowledge and skills to remove
entrapped victims from vehicles safely and efficiently preventing death and
disability that results from improper or prolonged extrication rescues.

How my life changed in a quick heartbeat!
This past February, I learned that I had a tumor on my pancreas. Doctors
informed me that immediate surgery was necessary. Here I was, in a situation
where I had to rely on properly trained professionals to save my life. I never
thought something like this would ever happen to me, but it did. Several hours
in surgery, and many tests and post-operation procedures later, I am ecstatic to
report that I am cancer free and healthy – and I owe my life to the outstanding
team of well trained and educated medical specialists who cared for me.
During my stay in the hospital, there was plenty of time for me to reflect on
life. I thought about how quick my life could have changed, should I have had a
different diagnosis, or less than optimal treatment. I thought about my
children, family, friends and life. I kept thinking how fortunate I was to have
well trained medical personnel right there when I needed them.
Traffic crashes are the leading cause of death of children. Imagine for a moment
and picture your child, family, friend or even yourself trapped in a vehicle
crash with metal crushing down on you and your life is slipping away. If you
ever found yourself or family member in this situation, you would expect the
rescuers who arrive on scene to be highly trained, and educated on vehicle
extrication –just as my doctors were well trained in saving my life in the
hospital. Sadly, this is not the case. The vehicle extrication
training or certification that is currently required by the State of Wisconsin
is NONE.
It is inconceivable to think that in today’s society, rescuers are performing
live-saving vehicle extrications without a formal certification training program
in place. Each model year, as vehicle technology advances, the methods to
extricate victims lags further and further behind. Emergency personnel must be
properly trained to save those precious seconds that can mean the difference
between life and death. The current mantra of emergency personnel who arrive to
a crash victim site, is “get them out anyway you can.” This is not what I want
for you, my children, family, friends or myself. Wisconsin citizens should
demand to be recipients of creditable and accountable extrication services when
lives are in jeopardy.

As the winter weather is in full swing and winter driving poses increased risks,
I would like to introduce you to an organization that may save the lives of you
and your loved ones. Safe And Fast Extrication, Inc. (SAFE) is a non-profit
organization dedicated to saving lives and reducing the suffering of victims by
bringing qualified and certified rescue personnel to the scene of a vehicle
collision.
What exactly is extrication? In simple terms, it is the removal of vehicle
metal and debris to disentangle entrapped victims of vehicle crashes.
Traffic crashes are the leading cause of death for children. By the end of 2003,
fatal vehicle crashes reached an all-time high with over 803 deaths. As a result
of vehicle crashes in Wisconsin, it is estimated that someone is injured every
8.3 minutes. Yet, it is likely that rescuers without formal vehicle extrication
training or certification are performing life saving vehicle extrications daily.
The "get-them-out-anyway-you-can” method is not the proper way to perform
vehicle extrication rescue. Rescuers should be held to higher standards than
that. SAFE was founded by firefighters who wanted to create and set the highest
standards in vehicle extrication rescue techniques and technology.
Currently, the Medical College of Wisconsin and SAFE are conducting post crash
vehicle extrication research under the direction of Frank Pintar, PhD, and
Narayan Yoganandan, PhD, both Professors in Neurosurgery and are Directors of
the Neuroscience Research Laboratory. This innovative extrication research will
serve as an educational tool for rescuers to develop skillful vehicle
countermeasures in today’s rapidly changing vehicle design. It is vital that
this ongoing extrication research and development continue and will be the key
to design alternate time saving extrication methods to improve rescue
procedures…ultimately saving more lives.
Progress is being made, thanks to David Rufer, Daniel Furseth, Shawn Conners,
Neil Andersen, Jarrod Wellik, Tom Wher, James Olshanski and Phil Bochler,
extrication specialists from Minnesota, Illinois, Indiana and Wisconsin, among
others. We still have a long way to go before we can achieve our goals to bring
state-of-the-art vehicle extrication services for all communities throughout
Wisconsin.
